This assigned code is based on the extent of the physical exam, the complexity of the medical visit and the patients medical history. The doctor will also offer a diagnosis for this patient and the visit. The diagnosis will have an attached number called an ICD -9-CM.
Both the CPT code and ICD 9 - CM along with the claim are then submitted to the insurance company by the Medical Billing specialist. The insurance company will then have their medical director review the claim. This review will determine the legitimacy of the claim based on the patients eligibility, medical necessity and the providers credentials.
When the claim is authorized then payment, at a percent, will be made to the provider. If the claim is denied then a notice is sent to the provider with an explanation attached. This has to be researched, corrected and then resubmitted. This process can be repeated many times until the insurance company agrees to pay or the provider agrees to accept little or no payment for the service. Due to the progressive complexity of claims and data entry errors, it is not uncommon to see claims denied about 50% of the time.
Because of the complexness of many claims and the fact that providers both want and need all of their entitled reimbursement, it is very important the the medical billing person be a specialist in this area. Economic slowdown seems to be affecting men and women differently, in line with their conventional social roles. The pressure of being the breadwinner is so high in the male psyche that currently 27% of them are living in constant fear of losing their job and more than 40% are tense regarding their financial status.
This has led to close to 3 million men in the UK suffering from various stress-related mental ailments like depression. The problem is aggravated because men refuse to talk to anyone about it, thus suffering in silence. It was found in a survey by Mind, a mental health charity that 69% men refused to talk to their families about their mental problems. Majority of them were also uncomfortable talking to their friends about it. However, the most worrying finding was that men seem to refuse medical treatment for these problems much more often then women do.
The stress level was lower in case of women, with only 22% of the respondents in the survey being worried about their job. Around 50% of them were open to talking to their families about these problems.
As the problem is more aggravated and complicated in case of men, Mind has given ten recommendations to deal with the situation. It has urged for special attention to be given to stress related problems in men and for a two pronged approach - action from both employers and the government in helping these employees.
